Location Call Center at BlueHen in Dover Status Full Time 80 Hours Shift Day/ Evening Salary Range 23.90 - 35.85 HOURLY General Summary Provides professional nursing care to patients within Physician’s Practice. Professional nursing practices, requiring specialized knowledge, judgment, and skill are performed in accordance with practice policy, procedure and philosophy. Note: This is a Phone Triage LPN position, located onsite at our Call Center at BlueHen. The LPN in this role will support our Bayhealth Primary Care offices and will typically not engage in direct patient care; they will instead be responsible for triaging phone calls, addressing patient inquiries and concerns via phone and the online patient portal. Responsibilities Perform triage services for patients. Prepare injections as required and according to treatment plans designed by the physician. Review medication refills with patient via telephone, ensuring periodic assessment in the office by the physician to determine therapeutic effect and patient compliance; adjust patient medications as needed. Assist with minor medical/surgical procedures in the practice setting. Recognize abnormal lab results; review lab reports with patients in the absence of the provider. Coordinate further testing and evaluation with the physician. Identify the need for patient education and advise patients on their treatment plan, including Diabetes and other chronic diseases. Maintain treatment records/files for each patient and complete patient encounter form at the conclusion of each service performed. Escort patient to the check‑out station at the conclusion of each service. Return patient phone calls regarding testing and/or treatment plan. Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice. Manage the patient treatment area: ensure compliance with Joint Commission regulations and safety requirements; dispose of used sharps and contaminated materials, and safely store medications. Re‑order supplies as required and within budgetary limitations. Submit requests for capital equipment during the annual budget development process. Maintain workstation cleanliness and instrument/equipment readiness and sterilization. Evaluate own nursing practice in relation to professional practice standards, statutes, and regulations, and engage in annual self‑assessment. Seek constructive feedback and take action to achieve performance goals. Participate in ongoing educational activities to acquire and validate knowledge (e.g., CPR, CLIA training programs). Maintain professional affiliations and attend staff meetings as required. Perform tasks and projects assigned by the Practice Manager or Physician, and serve as backup for other practice staff members as needed. Contribute to the professional development of peers and other healthcare providers through knowledge sharing and constructive feedback. Contribute to a team environment that encourages each member's contribution. Become proficient and cross‑trained in the front‑desk area to maintain front desk operations, including answering phones and filing. Perform all other duties as assigned within the scope of job responsibilities. Required Education, Credential(s) and Experience Education: High School Diploma or GED Certificate, Licensed Practical Nurse (LPN), or equivalent credit hours in an RN program. Credential(s): Licensed Practical Nurse (LPN) and Basic Life Support (BLS). Experience: 2 or more years of LPN patient care experience (new graduates may apply). Preferred Education, Credential(s) and Experience Education: Associate’s Degree. Credential(s): Advanced Cardiovascular Life Support (ACLS) Certification, if applicable. Experience: Two (2) or more years of clinical nursing & patient care experience in an inpatient or physician practice setting. #J-18808-Ljbffr
