RN: Case Manager

Job Description Join a focus‑oriented team as a Registered Nurse in Case Management, where you’ll be integral in coordinating patient transitions across multiple acute care departments. This contract offers stability with a full 13‑week assignment, working eight‑hour days from 8:00 AM to 4:30 PM, Monday through Friday, with two weekend rotations. You’ll primarily stay in the same area, providing discharge planning and case management support with patient ratios of 1 to 16‑20. The role involves meeting with patients and families to discuss care plans, utilizing EPIC for charting, and collaborating with support staff like social workers and utilization review RNs.
