Physical Therapist Assistant (PTA) - Skilled Nursing Facility

Job Description
We are seeking a compassionate and dependable Physical Therapist Assistant to join a Skilled Nursing Facility in Petaluma, California This position offers the opportunity to work with a diverse geriatric and post-acute population, assisting patients in recovering from surgery, illness, or injury. Under the supervision of a licensed Physical Therapist, you will provide hands-on care focused on improving mobility, strength, and overall functional independence. Job Responsibilities:
•Assist the Physical Therapist in implementing individualized treatment plans in a Skilled Nursing Facility setting
•Provide therapeutic exercises, gait training, and functional mobility activities
•Monitor patient progress and communicate any changes or concerns to the supervising Physical Therapist
•Educate patients and caregivers on exercises, safety, and proper mobility techniques
•Maintain accurate and timely documentation of treatments and patient progress
•Ensure compliance with facility policies and regulatory requirements

Requirements
•Active state license as a Physical Therapist Assistant in California
•Current BLS/CPR certification
•Minimum of 6 months of recent PTA experience within the last 5 years preferred
•Experience in a Skilled Nursing Facility or geriatric setting is a plus
•Strong communication and teamwork skills
