Pharmacist-Clinical - Pain/Oncology Acute Care

Qualifications License / Certification: Current California Registered Pharmacist license Education: Pharm. D. degree Experience – Inpatient: Completed PGY‑1 residency or at least two years clinical hospital pharmacist experience or current Kaweah Health Pharmacy Resident. Experience – Outpatient: Completed PGY‑1 residency or at least two years clinical pharmacist experience or current Kaweah Health Pharmacy Resident. Preferred: Three years experience Department Specific Requirements: see job responsibilities for detailed duties. Job Responsibilities Essential responsibilities for the pharmacist include the following: Inpatient Areas For patients in the acute care setting, review orders for appropriateness, profile/validate and dispense medications, supplies or equipment orders accurately and efficiently. Follow federal and state law and approved hospital policies and procedures regarding medication use. Consult with other health care providers and information resources to optimize therapy and maximize safety for medication administration for assigned areas. Use PYXIS units on the nursing units. Responsible for answering all inquiries from physicians and other staff regarding medication administration issues. Assure compounded medications meet standards for quality when performing or supervising compounding, performing quality reviews of the finished product and completing recordkeeping in accordance with department policy and procedures. Guide, direct, and assist in training pharmacy technicians, new pharmacist personnel, interns, and/or residents, assuming responsibility for quantity and quality of their work. Provide training professional education in the form of in‑service training, platform presentation, didactic lecture or hands‑on learning activities to other healthcare providers, trainees and/or staff when appropriate. Participate in the transition of care process of targeted patient populations through services such as medication reconciliation, medication education, and care coordination in collaboration with the interdisciplinary team. Monitor and assess patients’ medication and nutritional therapies, maintain their clinical charts with complete and timely documentation; intervene in patients’ care when indicated to optimize pharmacotherapy, including management of medication therapies following hospital approved protocols and policies. Offer cost‑effective and safe alternatives when appropriate. Participate in interprofessional patient care rounds where applicable. Set and review annual goals with manager. Outpatient Areas For patients in the outpatient setting, review prescriptions for appropriateness, profile/validate and dispense medications, supplies, and/or equipment orders accurately and efficiently. Follow federal and state law and approved hospital policies and procedures regarding medication use. Consult with other health care providers and information resources to optimize therapy and maximize safety for medication administration for the patient. Responsible for answering all inquiries from physicians and other staff regarding medication issues. Guide, direct, and assist in training pharmacy technicians and new pharmacist personnel, interns, and/or residents, assuming responsibility for quantity and quality of their work. Monitor and assess patients’ medication therapies and maintain their medical record with complete and timely documentation; intervene in patients’ care when indicated to address potential drug interactions and to optimize pharmacotherapy. Provide patient‑specific dosing and monitoring information to patients and providers. Provide consultation and actively participate in collaborative management of referred patients. Offer cost‑effective and safe alternatives when appropriate. Provide in‑service training to other healthcare providers and staff when appropriate. Set and review annual goals with manager. Addendum (essential for specific department) IF ASSIGNED TO HOSPITAL ALSO RESPONSIBLE FOR: Making outpatient prescriptions in accordance with federal and state law and hospital policies. Counsel patients upon receipt of prescription on appropriate use, storage and potential adverse reactions. Responding to Emergency Alerts (e.g. Code Blue, Code White, Code Sepsis, Code Stroke, Cardiac Alert) as assigned as a top priority. Preparing all dilutions, IVs, and other preparations ordered by the physician or approved by hospital protocols. Anticipating patient drug needs and responding accordingly. Filling all narcotic requisitions presented by technicians after verifying correctness of requisition. Maintaining all appropriate records demanded by federal and state laws and KH procedures. Conducting monthly nursing station inspections for cleanliness, orderliness, and drug inventory compliance. Organizing and planning monitoring of drugs encompassed by pharmacokinetics following all department procedures. Consulting with physicians on optimal drug therapy. Monitoring patients following department procedures and pricing patient charges under manager direction. Specialty Areas Applicable Emergency Medicine, Emergency Medicine Mid‑Shift Oncology, Infectious Disease, Anemia Management, Pain Management, Critical Care, NICU/Pediatrics. Serve as clinical expert in the area of specialty, integrating advanced knowledge and training to ensure desired service and patient outcomes. Participate in the care of and review/profiles medication orders for patients admitted and awaiting an inpatient bed. Coordinate with other pharmacists during high census. Document clinical interventions and measure clinical/economic effectiveness of services, reporting data at defined intervals set by management. Participate in the formulary process, prepare and present drug monographs, perform medication use evaluations, and develop and implement policies, order sets, and institutional guidelines. Act as pharmacy representative on institutional committees as assigned. Formulary Management Pharmacist Specialist Create and maintain a closed formulary of medications, collaborating with medical staff and Pharmacy & Therapeutics Committee. Review formulary addition or deletion requests and make formal recommendations. Facilitate development and implementation of treatment guidelines, protocols, formularies changes, and critical pathways. Monitor and address medication shortages, design alternatives, implement rationing, and collaborate with stakeholders. Oversee medication reviews and use evaluations, conduct periodic reviews, and recommend cost‑effective medication use. Provide education and training to pharmacy staff and facilitate training for nursing and medical staff. Develop and implement policies and procedures, report on strategic planning outcomes at defined intervals. Compounding Pharmacist Specialist Responsible for ensuring quality of compounded preparations within units and compliance with regulatory and best practices: Participate in development and maintenance of policies, procedures, and workflows. Oversee training, education, and competency assessment for pharmacy personnel. Develop and maintain Compounding Quality Assurance Program, ensuring documentation and reporting. Review quality assurance records for outsourced compounding pharmacies and prepare annual quality reports. Develop environmental, personnel, and performance monitoring programs. Lead continuous process improvement initiatives for safety and survey readiness. Compounding Pharmacist at the Outpatient Oncology Infusion Center Assure compounded medications meet standards for quality, perform quality reviews, and maintain recordkeeping in accordance with department policy. Ambulatory Care Pharmacy Specialist Provide direct patient care services with prescriptive authority via collaborative practice agreements. Order labs and assessments, provide medication therapy management services, advise prescribers on high‑risk medication profiles. Offer patient education and coordinate pharmacotherapy needs at discharge. Participate in multidisciplinary patient care rounds and develop population management programs for high‑risk patients. Document clinical interventions, measure outcomes, and report data at intervals set by management. Guide, direct, and train pharmacy technicians as patient care managers. Act as primary ambulatory care preceptor for residents and interns. Represent pharmacy on various committees, including the Integrated Delivery Network Clinical Oversight Committee and the Chronic Disease Management Center Committee. Family Medicine Pharmacy Specialist Provide direct patient care and prescriptive authority for referred patients via collaborative practice agreements. Recommend and order labs for monitoring and managing pharmacotherapy. Deliver didactic lectures and case discussions for physicians, residents, and staff. Identify opportunities for improvement in pharmacotherapy and provide education, training, and clinical services. Document interventions and quantify effectiveness, reporting data at defined intervals. Participate in research projects and committee duties; serve as preceptor for residents and students. Additional Responsibilities Demonstrate knowledge and skills necessary to provide care appropriate to the population served. Be knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age‑specific needs. Communicate effectively and care for patients and families as reflected in the Plan for Provision of Care. Perform other duties as assigned. Pay Range $72.90 – $109.36 #J-18808-Ljbffr
