Hospice LPN Continuous Care (PRN)

This position is based in Hialeah, FL and requires daily travel within an assigned territory in Miami Dade County. Schedule: Evening Shift/Flex Schedule. Collaborate with physician and RN frequently to ensure patient needs are being met; notify them as soon as possible when patient condition changes. Teach patients and caregivers self‑care techniques. Check vital signs. Review all medications and perform a medication reconciliation at each visit. Perform specialized procedures ordered by physician (e.g. foley catheter change). Prepare equipment and materials for treatments using the aseptic technique. Complete documentation timely and accurately. Qualifications

• *Bilingual in English/Spanish REQUIRED**
• *One year of clinical LPN experience REQUIRED**

Current LPN license, specific to the state(s) you are assigned to work Current CPR certification Valid driver's license, reliable transportation, and liability insurance Preferred Qualifications Hospice or home health experience Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities and patients' homes Proficient in electronic medical record (EMR) systems Ready to take the next step?
