Diabetes Nurse Educator

Hartford Healthcare seeks a compassionate Diabetes Nurse Educator to provide evidence-based diabetes self-management education for patients and families. In this role, you will assess learning needs, develop individualized education plans, and lead group classes on glucose monitoring, nutrition, and medication management.
