RN , Team Manager - Hospice Care

The RN, Team Manager (Hospice Care) evaluates and coordinates the various members of the interdisciplinary team to assure seamless continuity of care from admission to discharge. This leadership role ensures that hospice philosophy, regulatory compliance, and clinical care are aligned to meet the physical, emotional, and palliative needs of patients and their families. Responsibilities Manage patient care services across the interdisciplinary team to guarantee that clinical services reflect the established plan of care Coordinate advanced palliative care measures for pain and symptom management in close alignment with both the Attending Physician and Medical Director Oversee the insurance re-certification process by ensuring complete, accurate documentation is compiled and decisions are made within required regulatory timelines Review all prospective live discharges for extended prognosis to ensure comprehensive and effective discharge planning according to established hospice protocols Audit the type, frequency, and level of team services provided—including staffing, medications, DME, and supplies—to maximize quality and responsiveness to families Facilitate interdisciplinary team meetings, driving collaborative input to revise care plans in concert with the evolving needs of the patient Participate in the Administrative On-Call rotation and contribute actively to Quality Assessment Performance Improvement (QAPI) initiatives Partner with the clinical administration to lead hiring interviews, onboarding, training, supervision, and annual performance evaluations of team staff Serve as a direct clinical resource and mentor for personnel, troubleshooting inter-team issues and modeling excellent customer service standards Oversee team schedules and caseloads to ensure workload is distributed equitably and overall productivity expectations are consistently met Support community outreach efforts and maintain a highly collaborative, positive, and solution-oriented work environment free of negativity Requirements Current, valid California Registered Nurse (RN) License At least 1 year of active, hands-on professional nursing experience within the last 3 years Minimum of 3 years of hospice supervisory or management experience is highly preferred Deep understanding of hospice philosophy, general principles of palliative nursing care, and the interdisciplinary team process Proven ability to effectively supervise, direct, and develop both professional clinical staff and administrative personnel Strong multitasking capabilities paired with exceptional listening, communication, and interpersonal problem-solving skills Flexibility to operate in a full or hybrid telecommuting capacity at the discretion of executive management Valid California driver’s license with proof of automobile insurance Physical Demands & Work Environment Desk and field-balanced role involving sitting (60%), standing (20%), and walking (20%). Requires frequent reaching above the head (5%) and occasional lifting of equipment/supplies up to 5 lbs. Highly technical workspace utilizing computers, cell phones, and tablets for more than 4 hours per day #J-18808-Ljbffr
