Nurse Practitioner

Nurse Practitioner (NP) | Hands of Hope Physical Therapy Queens & Bronx, New York City | Outpatient Rehabilitation At Hands of Hope Physical Therapy , we are on a mission to deliver exceptional rehabilitation care while fostering a supportive and collaborative workplace. We are seeking a compassionate, motivated, and patient-focused Nurse Practitioner (NP) to join our growing team in Queens & Bronx. Responsibilities: As an Nurse Practitioner at Hands of Hope, you will: Patient Assessment & Diagnosis: Conduct comprehensive virtual evaluations, review patient health histories, assess physical diagnostic tests, and identify pain sources to establish targeted treatment plans. Treatment & Management: Develop individualized pain management plans; prescribe and manage medications (including non-opioid analgesics and adjunctive therapies) in compliance with regulations; assist with interventional procedures if credentialed; and educate patients on pain management strategies and physical therapy. Collaboration & Coordination: Partner closely with physical therapists and specialists to deliver integrated patient care, manage appropriate referrals, and participate in interdisciplinary team case discussions. Regulatory Compliance & Documentation: Maintain accurate, timely medical records within the Electronic Health Record (EHR) system; strictly adhere to federal and state guidelines for prescribing controlled substances; and follow organizational policies for safety and quality. Education & Professional Development: Stay current with evolving pain management treatments and clinical guidelines while completing all required continuing education programs. Qualification: Required: Master's or Doctorate degree in Nursing (MSN/DNP) from an accredited program. Active, unrestricted Nurse Practitioner license in NY; Board Certification (FNP, AGNP, or PMHNP preferred); active DEA registration with controlled substance authority; and current BLS/ACLS certification. Preferred: Minimum of 1 year of clinical experience in pain management preferred; experience with interventional pain management procedures is a plus. Expertise in acute and chronic pain management principles, strong clinical decision-making, proficiency with EHR systems, and excellent patient education and interpersonal skills.
