We proudly serve patients across Hampton Roads and are expanding rapidly. We're looking for a motivated, energetic professional to join our team as an Entry-Level Home Health Sales Representative.

Position Summary

The Entry-Level Home Health Sales Representative is responsible for promoting Elevate Healthcare Services' programs-including Skilled Nursing, Therapy, Orthopedic Recovery, Cardiac Care, Stroke Rehab, and Fall Prevention-to referral partners throughout the region. This individual will help increase brand awareness, generate referrals, and build strong partnerships. This opportunity provides mentorship and training from a sales leader with over 20 years experience in the industry.

Key Responsibilities

• Build and maintain relationships with physicians, case managers, hospital discharge planners, skilled nursing facilities, and community partners.

• Educate referral sources on Elevate Healthcare's services, specialty programs, and patient benefits.

• Conduct routine outreach visits, presentations, and community engagement events.

• Track leads, referrals, and activities using CRM tools or agency systems.

• Coordinate with the clinical team to ensure smooth onboarding and communication about referred patients.

• Represent Elevate Healthcare Services professionally to increase brand visibility and community trust.

• Support marketing efforts by distributing brochures, attending health fairs, and participating in networking events.

• Meet weekly activity goals and contribute to monthly census growth targets.

Qualifications

• High school diploma or equivalent; associate or bachelor's degree preferred but not required.

• Strong interpersonal and communication skills.

• Comfortable speaking with healthcare professionals and community partners.

• Reliable transportation (mileage reimbursement provided).

• Positive attitude, coachable, and eager to grow in healthcare sales.

• Highly organized, dependable, and confident working independently.

• Previous customer service, outreach, or similar experience is a plus.
