Youth Diversion Case Manager
Wheeler Health
Wheeler Clinic is seeking a dedicated Diversion First staff member to support youth diverted from formal juvenile court processing. The role focuses on coordinating referrals, providing direct case management, and building strong partnerships with police, schools, probation, DCF and service providers to meet youth needs and improve outcomes. Responsibilities include gathering histories, developing individualized plans, maintaining documentation for Medicaid/licensing, and supporting #J-18808-Ljbffr Wheeler Health
