We are looking for compassionate and skilled nurses to deliver exceptional care in a home setting, ensuring comfort and support for our clients and their families. Day Shift: Mon-Friday 12pm-8pm Responsibilities: Conduct comprehensive assessments of patients' health and care needs. Develop and implement personalized care plans in collaboration with healthcare teams. Administer medications and treatments as prescribed by physicians. Monitor and document patient progress and respond to changes in condition. Educate patients and families on health management and home care techniques. Coordinate with interdisciplinary teams to ensure holistic patient care. Maintain accurate medical records and adhere to compliance regulations. Provide emotional support and guidance to patients and their families. Requirements: Valid RN license in the state of California. Minimum of 1-2 years of nursing experience, preferably in home health or community settings. Strong assessment, critical thinking, and communication skills. Ability to work independently and manage time effectively. Current CPR and First Aid certification. Compassionate demeanor with a patient-centered approach to care. Proficient in electronic medical records (EMR) systems. Willingness to travel to patients' homes within the service area.
