Summary This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Application Information This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 10, 2026. The initial cut-off date for referral of eligible applications will be May 25, 2026, with subsequent cut-off dates every 2 weeks. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available. Location & Duties NOTE: This position is based out of the EUREKA VA Outpatient Clinic - duties entail traveling to the homes of Veterans living in the surrounding area. The Home Based Primary Care Nurse Practitioner (HBPCNP) at the Eureka VA Outpatient Clinic provides comprehensive, patient-centered primary care to a diverse Veteran population in an outpatient setting. Working within the VA's Patient Aligned Care Team (PACT) model, the PCNP delivers high-quality, evidence-based care focused on disease prevention, health promotion, chronic disease management, and coordination of services across the continuum of care. Given the rural and geographically isolated setting of Eureka, the HBPCNP plays a critical role in ensuring access to timely, continuous primary care services. The position requires strong clinical judgment, adaptability, and a commitment to serving Veterans in an underserved region while upholding VA standards of care, performance metrics, and regulatory requirements. The incumbent independently manages a panel of patients with acute and chronic health conditions, with an emphasis on health promotion and disease prevention using evidence-based practice in the home of Veterans. Professional Responsibilities Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of Integrity, Commitment, Advocacy, Respect, and Excellence. Other duties assigned. Qualifications & Requirements Valid Driver's License and auto insurance required. 1 full and unrestricted license from any US State or territory. No previous participation in EDRP (former participants are ineligible).
