Field Registered Nurse (RN) – Home Care – Greene County, NY | $85K

At Home Care Partners is growing our clinical team and looking for a dedicated Home Care Field RN to serve patients throughout Greene County, New York .

Our Field Nurses provide an important connection between patients, families, caregivers, and our clinical team. You'll spend your day in the community completing assessments, supporting aides, reviewing care needs, and helping clients safely receive care at home.

Responsibilities include comprehensive nursing assessments, head-to-toe evaluations, medication reviews, aide supervision, Plan of Care development and updates, caregiver guidance, monitoring changes in condition, clinical documentation, and communication with families and office staff.

Qualifications: Active NYS RN license, Medical Professional Liability insurance, ability to travel throughout Greene County, strong documentation and assessment skills, proficiency with Gmail and Microsoft Word/Excel, and the ability to work independently.

Home care, outpatient, or community nursing experience is preferred. Experience with Medicaid, Medicare, MLTC plans, and Plans of Care is also beneficial. Spanish/English bilingual candidates are encouraged to apply.

Salary: $85,000 annually
