ACT Case Manager

Description

Valley Community Services Board is seeking a dynamic individual to join our Assertive Community Treatment Team. The ACT Generalist provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. The incumbent will be working with a population of adults who may be dually diagnosed with chronic mental illness and substance use issue. Services are provided in accordance with the ACT model of treatment. Services are provided in multiple settings, including in the home and in the community.

The ACT Generalist position is full-time and requires working a schedule rotation of weekends and holidays. There are limited on-call responsibilities.

Summary of Essential Functions

• Provides the ACT Team model of assertive community treatment to include: Supportive interventions targeted at skill restoration and promoting overall psychiatric stability by addressing and coordinating supports in psychiatric rehabilitation, housing, vocation, substance abuse, and peer support.
• Ongoing client assessment, outreach, and advocacy.
• Crisis intervention including during scheduled on-call times.
• Routine community and home visits to assist consumers as defined by their individual treatment plan.
• Meets requirements for documentation of client service delivery and maintains records in accordance with agency and regulatory standards.
• Assists individuals in maintaining independent living.
• Provides emergency/crisis intervention and coordinates for hospital pre-screening.
• Assists in the provision of medical services, such as delivering and monitoring medications.

Requirements

Minimum Requirements

• Bachelor's or master's degree in human services related field.
• 1 year of direct clinical experience with SMI and dually diagnosed populations.
• QMHP or QMHP-T eligible.
• Availability to work weekend and holiday coverage.
• Availability to work in on-call coverage Monday through Friday.
• Experience with crisis intervention skills.
