(eligible after 1yr of 1000 hours worked) /n /n /n Our Culture /n /n - Leadership is available to confer, consult and advise at any time /n - You are encouraged to speak professionally and freely /n - Work-life balance is a top priority /n - Laughter, passion for personable-care, and a supportive environment /n /n /n RN Responsibilities /n /n - Complete ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family /n - Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician /n - Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment /n - Ensures that equipment, supplies, drugs and biologicals, and other necessary items and services are available /n - Make necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change /n - Completes accurate and timely documentation of patient/family services provided /n - Regularly attends IDG meetings /n /n RN Requirements /n /n - RN with active, unrestricted license in the State of Michigan /n - At least one (1) year of relevant experience as an RN (required) /n - One (1) year of management experience (preferred) /n - One (1) year of home health experience (preferred) /n - Current CPR Certification /n - Community health and a wide range of Medical-Surgical experience (preferred) /n - Reliable transportation with a valid state driver's license and current car insurance /n /n /n #Homecare /n /n Interim HealthCare is an Equal Opportunity Employer.
