ID: 63029116 Shift: AM Mon-Fri 8a-4pm , No WeekendsHome Health Experience RequiredCost center: 106500465812 5x8 Hr Description: Job Responsibilities:--Performs Patient assessments:-Accurately assesses patient s appropriateness for home care, learning needs and progress toward goals.-Collaborates with patient, family, and physician to develop the plan of care for the patient.-Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.--Maintains safe environment for patients and co-workers:-Assesses social environment and adjust or provide education as needed.-Assess for potential clinical risks and adapt program as indicated.-Follows universal infection control measures.--Implements treatment program to achieve designated goals:-Administer occupational therapy services per physician orders.-Instruct in communication and safe swallow, cognitive training as indicated.-Develop and instruct in therapeutic exercise program.-Implement program to restore function, and achieve maximum functional status.-Provides discharge planning and referral to resources as needed.-Communicate with physician, nurse, and other clinical disciplines as necessary.-Develops/implements/evaluates plan of care based on evidence-based knowledge.-Revise care plan as indicated within physician orders.--Uses age appropriate teaching methods for patients and families:-Evaluates teaching plan and revises as necessary.-Documents patient education tools utilized.--Demonstrates compliance with agency policies:-Assumes responsibility for Case Management.-Demonstrates knowledge of home care regulations and insurance reimbursement guidelines.-Assures compliance with regulatory requirements.-Follows agency documentation policies.-Shows evidence of interdisciplinary communication.-Ensures appropriate utilization of services.-Follows scheduling process.-Maintains visit productivity standards.-Participates in Team and IDT meetings.-Prioritizes schedule to accommodate patient's needs.-Travels to patient homes and facilities as indicated.-Complies with paid time off request process and weekend/holiday requirements.--Assists with growing agency business:-Provides orientation to new employees.-Accepts other assignments as requested.-Cross trains to learn new skills.-Use of personal vehicle is required for home visits.
