DUTIES AND RESPONSIBILITIES:

The Primary Care Nurse Practitioner (PCNP) delivers comprehensive primary care by actively collaborating with other healthcare team members, like behavioral health specialists, to address a patient's physical and mental health needs holistically, focusing on coordinated care and addressing social determinants of health within a single practice setting. PCNPs provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care, and health maintenance to patients. They serve as leaders and the point of contact for a patient's overall health while facilitating communication and collaboration across the care team to ensure that each patient gets the best possible care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Participates as an active member of the medical staff and participates in daily care team huddles to ensure prioritization of patient needs.
• Attends to patients in a timely and orderly manner to keep waiting time to a minimum and maximize productivity.
• Verify/update client information to include contact details, demographics, forms, consents, etc. at the start of every engagement. 
• Conduct initial evaluations, diagnose medical conditions, and develop treatment plans for patients.
• Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments.
• Reviews incoming reports (such as lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
• Screen patients for mental health concerns, provide brief counseling sessions, and refer to specialized behavioral health providers when necessary. 
• Collaborate with other healthcare team members, including nurses, social workers, and specialists, to ensure coordinated care and seamless transitions between services.
• Provide health information and education to patients regarding their conditions, treatment options, and preventative care strategies.
• Ensures documentation is complete in a timely and orderly manner, utilizing the electronic medical record.
• Actively participate in team meetings to discuss patient cases, share information, and coordinate care plans.
• Adheres to GCLC’s productivity and quality standards and meets minimum requirements.

• Maintain confidentiality and protect GCLC by abiding by laws and principles related to confidentiality; keep information concerning GCLC’s operations, patients, and employees confidential.
• This position requires compliance with GCLC’s compliance standards, including its Standards of Conduct, Compliance Program, and policies and procedures. Such compliance will be an element considered as part of the PCNP’s regular performance evaluation.

EXPERIENCE AND SKILL REQUIREMENTS: 

• Willingness and ability to lead medical staff committees or initiatives.
• Strong patient focus.
• Ability to manage multiple priorities.
• Ability to deal with trauma situations and death.
• Ability to make difficult decisions.
• Strong aptitude for calculating correct medicine doses. 
• At least three years of experience as a Nurse Practitioner is preferred.

QUALIFICATIONS

• Graduate of a Nurse Practitioner program and hold a Nurse Practitioner License. 
• Must possess an unrestricted and current license to practice medicine in the State of New Mexico. 
• Two (2) or more years of experience working in behavioral health is preferred, but not required. 
• Licensure must be maintained during the period of employment. 
• Bilingual (English/Spanish) preferred but not required.

LEVEL AND DEGREE OF SUPERVISION

Supervised by the Chief Executive Officer and/or Medical Director and direction from the consulting pharmacist. Supervision to include one (1) hour per week and participation in case staffing, general, and team meetings.

PHYSICAL REQUIREMENTS

• Prolonged periods of sitting at a desk and working on a computer.
• Must be able to lift up to 15 pounds at times. 

CONDITIONS OF WORK

Hiring and promotion will be accomplished in compliance with the GCLC’s Personnel Policies and Procedures. This position is exempt under the Fair Labor Standards Act. This position requires a minimum of 40 hours per week generally Monday through Friday. Hours or days may vary according to need. Travel is required of employees for delivery of services, training, or conferences. Personal transportation is mandatory.
