Physical Therapist (M-F) Day shift

Physical Therapist

Qualifications:

• Bachelor's (B.A., B.S.), Master's (M.A., M.S.) or Doctorate (DPT) degree in Physical Therapy
• Physical Therapy board certification and licensure according to state regulations

Hours:

Full-time days: Monday-Friday 8:30 a.m. - 5:00 p.m.

Pay period: Monday through Sunday

Two recent employment references

Online verification of licensure or certification to include expiration date and any disciplinary actions within 30 days of start date

BLS - Copy of both sides of the card – through American Heart Association

Immunizations to include:

• MMR (Mumps, Rubeola & Rubella) either 2 doses or Positive Titers
• Varicella-positive titer or 2 Varicella Vaccines
• Polio
• Hepatitis B Series, titer or signed declination
• Current TDaP – Tetanus, Diphtheria and Pertussis (dated within 10 years of start)
• COVID-19 vaccine
• Proof of Flu vaccination during Flu Season (Oct 1st – March 31st)
• Negative 2-step Mantoux within 1 year of start or QuantiFERON Gold if Positive, must provide proof of positive & chest x-ray results
• Proof of physical within 1 year of start date
• 10 Panel Drug Screen within 1 month of start date
• Respiratory Fit Test within 1 year of start date (we can provide RFT if not available)
• Criminal Background (All states where they hold a professional license and have lived/worked in the past 7 years) within 30 days of start date
• National &/or State Sex Offender GSA(EPLS) Sam.gov within 30 days of start date
• OIG-Date within 30 days of start date
• Iowa Mandatory Reporter Certificates (Child Abuse & Dependent Adult Abuse)
• Completion of CareLearning education modules (further instructions will be provided by JCHC) Mandatory Reporter will be added to modules if the candidate is not certified. Must have completed prior to start.
