This position will initially be filled as a temporary full-time position, with the opportunity for consideration for regular full-time employment. The temporary assignment is anticipated to last approximately 90 days. Conversion to regular employment will be based on satisfactory job performance and attendance. Temporary-to-hire status does not guarantee conversion to regular employment.

A challenging and rewarding opportunity awaits a new Quality Program Coordinator at Sante Health System in Fresno, CA. The Quality Program Coordinator supports the operational execution of Santé Physicians' Quality Management program by coordinating quality reporting, data preparation, member outreach initiatives, departmental and health plan partnerships, provider education, and day-to-day program operations. The position works closely with Quality Management team members and internal and external partners to organize information, coordinate activities, track progress, and support timely completion of quality initiatives.

QUALIFICATIONS

• High School Diploma/GED equivalent required.
• Doctor's office, IPA/MSO, or Health Plan experience preferred.
• Computer proficiency with Microsoft Office Suite, including a strong background in Excel, and the ability to use and learn company-specific systems as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Quality Reporting & Data Coordination

   • Prepare, format, organize, and distribute recurring quality reports, dashboards, care-gap lists, HCC lists, and other quality-related reports using Excel and established reporting tools.
   • Reconcile information across multiple reports and data sources and identify discrepancies for appropriate follow-up.
   • Collaborate with the Senior Quality Management Data Analyst to prepare data for provider engagement, member outreach, ACO activities, and health plan reporting.

2. Member Outreach & Quality Initiatives

   • Coordinate member outreach activities and targeted member lists for the Santé Care Center, Care Coordination, and other internal partners.
   • Track outreach activities, completion, and outcomes and support reporting to internal teams and health plan partners.
   • Coordinate assigned Quality Management initiatives, projects, and special programs from planning through completion.

3. Health Plan & Provider Support

   • Participate in health plan quality meetings; document minutes, coordinate follow-up, and track action items.
   • Receive and reconcile health plan care-gap and outreach reports with internal information and escalate discrepancies for review.
   • Support provider engagement through preparation of provider reports, care-gap information, HCC opportunity lists, education materials, and follow-up resources.
   • Coordinate new provider orientation and provider education activities.

4. Program Operations

   • Coordinate Quality Management events, health fairs, department and/or health plan partnership initiatives, and other community or provider activities.
   • Prepare and complete supplemental data uploads into Cozeva in accordance with established processes and training, and maintain supporting documentation.
   • Manage the Quality Management department phone and email and coordinate routine departmental communications, meetings, trackers, and follow-up.

5. Process Improvement & Team Support

   • Maintain organized documentation, project timelines, and tracking tools to support program accountability.
   • Identify opportunities to improve processes, communication, organization, and follow-through.
   • Collaborate with Quality Management team members and other departments to support organizational quality goals.
   • Other functions and accountabilities may be assigned to support business needs.
