Job Description & Requirements Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Westfield, New Jersey. Specialty Physical Therapist Discipline Therapy Start Date 05/18/2026 Duration 13 weeks Hourly and Weekly Hours 40 hours per week; 8 hours per day; shift: 8 hours; 36 guaranteed hours per week; scheduled 40 hours. Assignment Details Facility type: Skilled Nursing Facility. Shift: Days (08:30–16:30). Scheduled as 5x8 with a 30‑minute meal break. Guaranteed hours: 36 per week; scheduled 40 hours. Assignment duration: 13 weeks. Location: Westfield, New Jersey. Responsibilities Help patients recover from injuries and manage chronic conditions. Provide rehabilitation to adults with post‑acute and chronic conditions, including mobility and functional restoration. Evaluate and treat patients; document care in accordance with Medicare/Medicaid regulatory requirements. Work independently and coordinate care with interdisciplinary teams. Qualifications Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active New Jersey Physical Therapist license required to start the assignment. BLS certification from American Heart Association or American Red Cross (may be required). Experience working in a skilled nursing facility or long‑term care setting. Demonstrated ability evaluating and treating adults with post‑acute and chronic conditions, including mobility and functional restoration. Experience with documentation systems commonly used in SNFs and Medicare/Medicaid regulatory requirements. Ability to work independently and coordinate care with interdisciplinary teams. Preferred: prior travel PT experience and proficiency with restorative nursing programs. Compensation & Benefits Pay package is based on 8‑hour shifts and 40 hours per week (subject to confirmation) with tax‑free stipend amount to be determined.
