Position: – Home Health

Schedule: Full-Time / Part-Time / PRN

Compensation: Negotiable

Blue Water Staffing Solutions is looking for skilled Home Health Clinicians to join our

growing team. You’ll provide individualized physical therapy services to patients in the comfort of

their own homes while helping them improve mobility, independence, safety, and quality of life.

What You'll Do

● Perform comprehensive physical therapy evaluations in the home

● Develop and implement individualized treatment plans

● Provide therapeutic exercises, mobility training, gait training, and functional activities

● Educate patients and caregivers on home exercise programs and safety

● Monitor patient progress and modify treatment plans as needed

● Collaborate with physicians, nurses, occupational therapists, and other members of the

care team

● Complete required documentation accurately and timely

What We're Looking For

● Current, unrestricted Physical Therapist license in CA

● Graduate of an accredited Physical Therapy program

● Current CPR/BLS certification

● Home health experience preferred, but motivated new graduates may be considered

● Strong clinical, communication, and organizational skills

● Reliable transportation and valid driver's license

● Compassionate, professional, and patient-centered approach
