Job Summary
We are looking for a CNAs and PCAs to join our team! You will directly work with clients, following a one-on-one service plan in client's homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets clients needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Private Home Care or Home Health Care experience is a plus. 

Responsibilities 

• Assist clients with daily activities such as moving in and out of beds, baths, dressings, grooming, meal prepping, vitals, shopping, and wheelchair, or Hoyer lift assistance
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care needs
• Maintain records of daily tracking of client care, condition, progress, or problems to report and discuss observations with the supervisor 
• Build relationships with clients by conversing or reading aloud to help keep them mentally healthy and alert 
• Give medication reminders 
• Transporting, Scheduling and/or accompanying clients to medical appointments
• Engage patients in exercises or other activities  

Qualifications

• Current TB Skin Test required
• Driver’s license required 
• BLS CPR certification required
