Office Location: 6600 France Avenue South, Ste. 210, Edina, MN 55435
Coverage area: Hennepin / Carver / Scott Counties

Schedule:  Mon-Fri, 8:00 AM - 5:00 PM

As a RN Case Manager You will: 

Assess/monitor physical, emotional, and psychological needs of patients

Create hospice care plans that align with the patient's wishes and goals

Direct nursing care: administering medications, treatments, and interventions

Provide pain and symptom management

Educate and support the patient’s family and caregivers

Collaborate with interdisciplinary team

Maintain accurate and timely documentation
