Job Overview

We are in search of an enthusiastic and mission-driven Lead Physician for Primary Care in Dayton. This role is pivotal in implementing team-based collaborative care and addressing the needs of our medically complex and socially vulnerable members.

Care Model Features

• Manage smaller patient panels, averaging 10-12 patients daily
• Engage in longer visits to foster deeper relationships
• Collaborate within an interdisciplinary team
• Focus on prevention, reduced healthcare utilization, and improved outcomes

Key Responsibilities

The Lead Physician will have four primary areas of responsibility:

1. Deliver exceptional primary care to a designated panel of members while leading the interdisciplinary care team.
2. Guide fellow providers in delivering high-quality primary, urgent, and community care, mentoring them during population health discussions to meet quality and utilization targets.
3. Serve as the clinical leader alongside the Medical Director, providing clinical guidance to nurses, medical assistants, and other providers on various clinical cases.
4. Be an integral part of the center and market leadership team, accountable for driving the market towards achieving quality, utilization, and engagement objectives. Participate in leadership meetings and represent the organization to community-based organizations, hospitals, and payer partners.

Additional Duties

• Deliver high-quality primary care, managing chronic conditions, acute exacerbations, and follow-up needs post-hospitalization or ER visits.
• Collaborate with an interdisciplinary team (including behavioral health, nursing, social work, pharmacy, nutrition, and care management) to develop and adjust patient care plans.
• Coach and mentor physicians and Advanced Practice Providers on best practices in value-based and evidence-based care.
• Evaluate when patients can be treated safely within our organization and when higher-level care is necessary.
• Accept same-day, next-day, and urgent appointments for members requiring immediate physician or care management intervention to prevent admissions or readmissions.
• Monitor patient health trends and proactively work to minimize ER visits, hospitalizations, and readmissions.
• Practice evidence-based medicine in alignment with Patient-Centered Medical Home (PCMH) principles, ensuring quality, prevention, and continuity of care.
• Develop and manage transitional care plans, tracking patients throughout the entire continuum of care.
• Achieve and maintain quality metric goals, utilizing data and electronic health record (EHR) tools to enhance performance and efficiency.
• Provide on-call coverage on a rotating basis.
• Dedicate approximately 70-80% of time to direct clinical care.
• Perform additional duties related to chronic care and disease management as necessary.

Qualifications

• Possess an active, unrestricted medical license in the state of employment.
• Have clinical and administrative experience in leading interdisciplinary teams.
• Experience in a clinic, community-based organization, or inpatient unit is preferred.
• Demonstrated experience in training and mentoring others.
• Strong problem-solving, project management, and organizational skills.
• Self-motivated and critical thinker with an interest in population health, healthcare utilization management, and the continuous improvement of our care model.
