Assignment Details Contract length: 12 weeks Shift length: 8 hour shifts Hours per week: 40 Start date: ASAP Job Responsibilities Communicate functional changes and barriers to the care team to support care planning. Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs. Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate. Work 40 hours per week with 8-hour shifts.
