Job Summary
- Advocate for the client inside and outside of Lesprit Inc. regarding needed services
- Maintain constant communication to ensure that all services are being monitored and advocate for the client if this is not happening
- Be able to engage with client and clients family and develop a professional therapeutic relationship with boundaries
- Be able to maintain calm and confident during a clients crisis and maintain safety for both client and staff
- Be able to follow plan for emergency evaluation and management of crisis
- Provide assessments and service plans for client and clients family
- Create engaging activities to enhance the client and clients family in a therapeutic setting
- Provide progress notes in a timely manner regarding all contact with the client or on behalf of the client. Any significant changes in the clients condition will be noted in a timely manner
- Be prepared for quarterly reviews for service assessment
- Assist in staff review, document employee disciplinary issues as defined by Lesprits policy, report to program supervisor and administration as necessary assist in staff review, document employee disciplinary issues as defined by Lesprits policy, report to program supervisor and administration as necessary
- Must adhere to Lesprit Inc. policy and procedure including productivity and case load expectations and ethical and confidentiality expectations while following the code of ethics
- Provide monthly supervision, face to face at least once a month with family support specialist, therapeutic adult mentor and home support specialist
- Follow PQI standards
- Be aware of personal mental health needs and model a healthy lifestyle
- Be responsible for additional responsibilities as needed or assigned by supervisor
Minimum qualifications for a Case Manager are an Bachelors degree in human services field with at least one year full-time experience serving people with mental illnesses. Individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary skills, may also be employed as intensive case managers. Valid Montana drivers license and good driving record are essential. Benefits
