Dialysis Social Worker - Patient Advocate on Care Team

DaVita is seeking a Social Worker to join the outpatient dialysis care team in San Diego, CA. You will support patients in navigating the physical, emotional, and financial demands of End Stage Renal Disease, while advocating for patient rights and responsibilities. You will build lasting relationships with patients and families in a collaborative, supportive environment and participate in quality improvement efforts to enhance patient outcomes. #J-18808-Ljbffr DaVit
