Job Description To continue to be an industry pioneer in delivering unparalleled care, we need a Registered Nurse Case Manager with commitment and compassion. Are you one of them? What is Required? Current State license as a Registered Nurse. Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing 1+ year of experience as a nurse in a clinical care setting. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Experience in hospice or home health environment is preferred What's in it for you?
