The role involves delivering specialized physical therapy care to geriatric patients in a home setting, with responsibilities including patient assessments, therapy planning, and documentation. Job Details Specialty: Physical Therapist Home Health (PT HH) Certifications: BLS (Basic Life Support) required Education Requirements: Valid license in hand for the state of practice Experience Requirement: Minimum of 1 year of relevant experience, including experience with OASIS (Outcome and Assessment Information Set) for staff completing Start of Care (SOC) and proficiency in Homecare Homebase (HCHB) required Location Requirements: Active drivers license and personal transportation are mandatory Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week Additional Requirements: All charting must be completed by 9 a.m. the following morning; timely communication and documentation are mandatory; RTO (Requested Time Off) must be submitted at the time of application Responsibilities: Providing skilled physical therapy services, patient assessments, documentation, and coordination within a designated territory up to a 60-mile radius from the hub/office Certification and Requirements: Certification in BLS, active drivers license, and flexibility with territory are essential Submission: Candidates must have an active license in hand and be prepared for Monday orientation. All travelers are required to have an active drivers license, and personal transportation.
