The Position As a Nurse Practitioner, you’ll be responsible for supporting patient monitoring across post-acute and skilled nursing facilities in Seattle and Ellensburg. In this in-person, flexible role, you’ll collaborate with interdisciplinary care teams to drive better patient outcomes using AI-enabled technology. This position allows for flexible scheduling to work when you want at various skilled nursing facilities located in Ann Arbor, Ypsilanti, and Canton, MI. Primary responsibilities include Conduct in-person patient assessments: Complete comprehensive health evaluations using clinical insights and AI-powered tools. Visit skilled nursing facilities and/or patient homes: Provide on-site support and collaborate with care teams. Develop individualized care plans: Leverage patient data and predictive analytics to guide clinical decisions. Ensure accurate documentation: Maintain compliant, timely records within the EHR system. Collaborate across teams: Partner with physicians, VRNs, and SNF clinical teams to coordinate patient care. Qualifications and Candidate Background Certified and licensed Nurse Practitioner in the state of Michigan, with ANCC (or equivalent) certification in Adult, Family, Geriatric, or Acute care. 2+ years of experience providing care to complex adult populations. Ability to practice independently with minimal supervision. Proficient with healthcare technology, including EMRs, Google Suite, Slack, and AI tools.
