The Clinical Pharmacist – Transitions of Care at Olathe Hospital provides comprehensive pharmaceutical care to ensure safe, effective, and cost-conscious drug therapy. You will collaborate with the care team and actively participate in rounds, medication reconcil and patient education to optimize outcomes.
