PHYSICAL THERAPIST

Reports to the Director of Wellness

RESPONSIBILITIES:

The Physical Therapist (PT) will work in coordination with clinic staff to treat patients to restore, maintain and promote overall fitness and health. The PT will coordinate with clinic staff and work with patients to restore function, improve mobility, relieve pain and prevent injury. The PT will work with clinic staff and patients to establish creative plans of care that reduce potential for injury and allow patients to improve their quality of life.

MINIMUM REQUIREMENTS:

• Doctor of Physical Therapy
• Physical therapist license
• 3 years of experience

RESPONSIBILITIES:

• Perform an initial exam and evaluate medical history to determine a diagnosis and treatment plan.
• Administer manual exercises, massage or traction to help relieve pain, increase patient strength or decrease or prevent deformity.
• Coordinate with clinic staff to ensure level of patient care.
• Record prognosis, treatment, response and progress in patient's chart.
• Discharge patient from therapy when projected outcomes have been attained and provide follow-up care.
• Other duties as assigned.
