Physical Therapist

Watertown Health Care Center is a skilled nursing home facility that values compassion, respect, and professional care. We are currently recruiting a Physical Therapist to support residents with impaired activities of daily living. Benefits of this Position Great work environment Same‑day pay with Tapcheck Job location: Watertown, WI Purpose of this Position The purpose of this position is to evaluate, treat, document, and facilitate care for residents whose ability to perform activities of daily living has been impaired. The Physical Therapist will participate in interdisciplinary team meetings and develop restorative programs to improve resident function. Essential Job Duties & Responsibilities Evaluate and document evaluations of referred residents in accordance with facility policies and procedures. Provide appropriate treatment to help residents maintain or attain the highest level of function possible within their physical abilities. Recommend and/or provide appropriate treatment, preventative measures, and adaptive equipment to prevent functional decline. Provide resident or caregiver education to support functional maintenance. Maintain timely and accurate resident reports required by federal, state, and local regulatory agencies and third‑party payers. Make recommendations to administration for adaptive equipment and preventative measures. Observe infection control procedures and ensure resident safety under supervision. Assume duties related to completion of Minimum Data Set assessments as assigned. Complete documentation related to evaluations, treatment plans, MDS assessments, resident care plans, and progress notes in a timely manner. Participate in resident care conferences, interdisciplinary conferences, and rehabilitation conferences as scheduled. Assist in implementation of programs to improve resident care related to quality indicators. Assume quality assurance duties as assigned. Collaborate with residents, families, staff, physicians, consultants, and ancillary service providers. Observe all facility policies, safety procedures, and the employee handbook, including compliance with the Corporate Compliance Program, nondiscrimination policies, and the privacy of resident protected health information. All other duties as assigned. Qualifications Excellent verbal and written communication and organizational skills; ability to read, analyze, and interpret medical information; effective communication with residents, families, and healthcare professionals; ability to present information to the supervisor. Strong reasoning ability, including problem definition, data collection, fact establishment, and conclusion drawing. Familiarity with Word and Excel software. Degree in Physical Therapy.
