Patients seen/consults per day: 5-8
Certifications Required: BLS, MSW, 5150
EMR System: Cerner
Weekend Requirement: Every Saturday
Holiday Coverage: Rotate with staff
Scrub Color/Uniform: White lab coat
Hospitalist or Intensivist in-house: Yes
Pharmacy: 24/7
Shifts: Wednesday to Saturday, 16:00 - 02:30
Coverage: Emergency Department and floating based on hospital needs
Orientation: Floor-specific orientation in Emergency Department

Qualifications

Master's degree from a school of Social Work accredited by the Council of Social Work Education
Minimum of two (2) years of clinical and/or medical social work experience
Excellent customer service and presentation skills
Strong interpersonal and written communication skills
Ability to apply analytical and problem-solving skills
Proficient in managing multiple tasks or projects effectively
Detail-oriented with the ability to work independently
Ability to thrive in a fast-paced environment with changing priorities

Responsibilities

Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
Provide patient and family-centered care that respects personal self-determination.
Conduct comprehensive assessments and develop treatment plans addressing medical, social, and resource issues impacting patients and families.
Collaborate with healthcare team members to implement patient-centered treatment plans.
Engage in crisis intervention and high-risk screenings, as well as brief counseling and resource referrals.
Coordinate family and patient representative meetings with the healthcare team as needed.
Assist with post-acute placement for complex discharges, engaging appropriate agencies or community resources.
Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, and community resources.
Participate in hospital committees and contribute to the development and presentation of in-services for staff.

Additional Requirements

Must obtain 5150 certification at the county level.

Cancellation Policy

The hospital reserves the right to cancel up to 4 shifts per 13-week assignment, and up to 3 shifts for assignments shorter than 13 weeks.
