Responsibilities:

• Provide direct patient care, including assisting with daily living activities, such as bathing, dressing, and grooming

• Assist with feeding, toileting, and ambulating patients as needed

• Monitor and report changes in patients' conditions to licensed nurses and other healthcare professionals

• Maintain accurate and detailed records of patient care and services provided

• Participate in resident care planning and implementation

• Collaborate with interdisciplinary teams to ensure comprehensive care

Qualifications:

• Current and valid Wisconsin State CNA Certification is required

• Previous LTC/Nursing home experience is preferred

• Must be dependable and have a strong work ethic

• Exceptional Customer Service skills

• Friendly and energetic demeanor
