Non-Medical Case Manager (Bilingual)

CentroMed is a 501(c)(3) non‑profit organization that provides comprehensive health and dental care to all populations across the greater San Antonio area. Job Summary Responsible for providing non‑medical case management for Ryan White patients, maintaining them in a primary medical care system to improve health outcomes. The Ryan White Non‑Medical Case Manager assists clients with obtaining medical, social, community, legal, financial, and other resources. Duties and Responsibilities Complete client intakes, needs assessments, and care plans upon initial contact; assist patients with PAP applications, ADAP, and other available resources. Maintain regular contact with clients and complete needs assessments and care plans aimed at client self‑sufficiency; empower clients to develop independent living skills. Advocate on behalf of clients to reduce service gaps and remove barriers. Provide resource contact information, coordinate, and complete referrals for services needed. Coordinate with community partner agencies to ensure timely service delivery. Document all client contacts daily in the client EMR, Take Charge Texas, and/or Ryan White database. Evaluate patients’ Ryan White eligibility, including dental referrals from other agencies. Make information about services available to the public and identify potential patients. Explain program services, policies, and procedures to patients and staff. Assist with medical outreach events in the community. Abide by infection control, hazardous materials, safety policies, and the agency’s quality improvement program. Perform other duties as required. Qualifications Minimum of a bachelor’s degree in social work or a related field. At least two years of case management experience in a health care setting. Good understanding of working with indigent populations. Own transportation and auto insurance. Current Texas state licensure. Physical Requirements Capable of performing the essential functions of the position. Meet attendance requirements. Not pose a threat to health or safety of others.
