Certified Nurse Practitioner (CNP) - Pediatric IDD & Senior Care

Location: Atlanta, GA Employment Type: Full-Time Overview A growing healthcare organization is seeking a compassionate and experienced Certified Nurse Practitioner (CNP) to provide medical care and clinical oversight for both pediatric patients with intellectual and developmental disabilities (IDD) and an adult/senior population receiving home and community-based services. This position offers the opportunity to make a meaningful impact across two underserved populations by delivering patient-centered care in home, community, and outpatient settings throughout the Atlanta area. Responsibilities Conduct comprehensive health assessments and ongoing evaluations for pediatric and adult patients. Diagnose and treat acute and chronic medical conditions. Develop, implement, and monitor individualized treatment plans. Prescribe medications and therapies in accordance with state regulations and organizational protocols. Collaborate with physicians, nurses, therapists, behavioral health professionals, caregivers, and interdisciplinary care teams. Provide care coordination for patients with complex medical, developmental, and behavioral needs. Support preventive care initiatives and health education for patients and families. Monitor patient outcomes and adjust care plans as appropriate. Maintain accurate and timely clinical documentation within the electronic medical record system. Participate in quality improvement and care management initiatives. Pediatric IDD Population Provide primary and preventive care for children with intellectual and developmental disabilities, including autism spectrum disorder and related diagnoses. Partner with families, therapists, educators, and behavioral health providers to ensure coordinated care. Address developmental, behavioral, and medical concerns while promoting overall wellness and quality of life. Senior Care Population Deliver primary and ongoing care to seniors receiving home-based and community-based services. Manage chronic conditions including diabetes, hypertension, heart disease, dementia, and other age-related health concerns. Conduct wellness visits, medication reviews, and care plan updates. Support aging-in-place initiatives and reduce avoidable hospitalizations through proactive care management. Qualifications Current Georgia Nurse Practitioner license in good standing. Certification as a Nurse Practitioner (FNP, PNP, AGNP, or related specialty). DEA registration (or ability to obtain). Minimum of 2 years of clinical experience preferred. Experience working with pediatric, IDD, developmental disability, home care, geriatric, or primary care populations strongly preferred. Strong assessment, communication, and care coordination skills. Ability to work independently while collaborating effectively with interdisciplinary teams. Valid driver's license and reliable transportation.
