As a Clinical Social Worker, your day-to-day responsibilities will encompass a range of tasks aimed at supporting the emotional and psychological well-being of our patients, including:

• Conducting thorough psychosocial assessments to evaluate patients' mental health status, identifying their strengths and needs.

• Implementing evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patient requirements.

• Collaborating with multidisciplinary teams—including psychiatrists and nursing staff—to ensure integrated and holistic care.
\n
• Providing crucial education to patients and their families regarding diagnoses, treatment paths, and medication management.
\n
• Facilitating individual, group, and family therapy sessions to build resilience and coping strategies among patients and caregivers.
\n
• Developing comprehensive treatment plans in alignment with the patient's personal goals and preferences, consistently reviewing and modifying these plans as needed.

• Advocating for patients in navigating the complexities of the mental health system, ensuring they have access to necessary resources and support.

• Leading therapeutic groups, psychoeducation sessions, and discharge planning meetings to prepare patients for next steps in their care.

You will work primarily with patients experiencing terminal illnesses, focusing on their psychological, emotional, and social challenges as they navigate their healthcare journeys. This role requires a delicate balance of empathy, understanding, and professional insight, allowing you to foster meaningful connections with patients and their families during a critically emotional time.

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• In-depth knowledge of mental health disorders, evidence-based treatment modalities, and crisis intervention techniques.

• Excellent interpersonal skills and the ability to collaborate effectively within a multidisciplinary team environment.

While not mandatory, experience working with hospice or palliative care patients and familiarity with related community resources will greatly enhance your effectiveness in this role. A background in grief counseling would also be advantageous, as you will be aiding patients and their families through one of life’s most challenging times.
