The Community Action Agency of Western CT (CAAWC)is seeking a Program Manager – Rural Health Transformation Program (RHTP) to coordinate its Regional Collaboratives project and partnerships supporting Connecticut’s Rural Health Transformation Program.

This is a highly collaborative position for someone who enjoys bringing people together, keeping complex projects moving, and working with community partners to improve access to services in rural communities.

What You’ll Do

• Coordinate the RHTP regional collaborative project, including managing timelines, priorities, and required deliverables.
• Build relationships with healthcare providers, state agencies, municipalities, nonprofits, businesses, and community organizations.
• Organize and facilitate collaborative meetings, workgroups, listening sessions, and community engagement activities.
• Track program progress, performance information, reporting requirements, and contract deliverables.
• Gather community and partner feedback to identify rural health needs, service gaps, and barriers to access.
• Work with agency leadership and Finance to support grant, budget, and contract requirements.
• Coordinate program outreach, presentations, newsletters, success stories, and other communications.
• Represent Access at regional and statewide meetings, conferences, workgroups, and community events.

What We’re Looking For

• Bachelor’s degree in public administration, public health, nonprofit management, community development, healthcare administration, social services, business administration, communications, or a related field. Significant directly related experience may be considered in place of the degree when permitted by funding requirements.
• Experience coordinating projects, programs, grants, community initiatives, or collaborative partnerships.
• Strong project management, organization, communication, and relationship-building skills.
• Ability to manage multiple priorities, deadlines, and partnerships.
• Comfortable facilitating meetings and communicating with a wide range of community and professional stakeholders.
• Proficiency with Google Workspace.
• Valid driver’s license and ability to travel throughout Connecticut as required.

Preferred Experience

Experience with government-funded programs, Connecticut state agencies, healthcare or human services, rural communities, regional partnerships, data collection, program evaluation, or grant reporting is preferred.

Position Details

• Schedule: Full Time
• Pay: $27-28 per hour
• Location: Danbury Office with regular travel to meetings and partner locations throughout Connecticut
• Reports To: Executive Director
• Funding: Connecticut Rural Health Transformation Program (RHTP)

Pay: $27.00 - $28.00 per hour

Expected hours: 35.0 per week
