Registered Nurse (RN) Care Manager – Pediatric Home Care Per Diem | Field-Based | Bronx County, NY Join a Premier Concierge Pediatric Home Care Team Affirmed Home Care is seeking an experienced, compassionate, and motivated Registered Nurse (RN) Care Manager to support pediatric clients throughout Bronx County, NY . This per diem field-based position is perfect for an independent nurse who values flexibility, autonomy, and the opportunity to build meaningful relationships with patients and families while delivering exceptional one-on-one care in the home setting. Position Responsibilities Conduct in-home nursing assessments and case management visits Develop, implement, and oversee individualized pediatric care plans Obtain and maintain physician orders in accordance with NYS regulations Monitor, document, and communicate changes in patient condition Perform supervisory visits for home care staff Collaborate with families, physicians, and interdisciplinary care teams Promote patient safety, comfort, independence, and continuity of care Qualifications Minimum of 2 years of recent RN experience in pediatric home care or acute care Strong pediatric clinical experience with: Ventilator and tracheostomy care G-tube/PEG tube care IV infusions Wound care management Active New York State RN License Current BLS Certification Recent physical exam (within 1 year) Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years) Valid MMR lab report Valid driver’s license and reliable transportation Authorized to work in the United States Compensation & Benefits Competitive Pay

• 150 per assessment
• 125 per aide supervision visit
• 55/hour
