In this role, you will take ownership of the RAI/MDS process while helping ensure accurate reimbursement, effective care planning, regulatory compliance, and quality resident outcomes.

As the RN/LPN MDS Coordinator , you will oversee the full Resident Assessment Instrument (RAI) process , ensuring MDS assessments are completed accurately and on time while supporting individualized care planning, regulatory compliance, and reimbursement accuracy.

This position is a great fit for an experienced MDS professional who is organized, deadline-driven, detail-oriented, and committed to clinical excellence.

• Coordinate and ensure timely and accurate completion of MDS assessments

• Oversee the RAI process and maintain compliance with applicable requirements

• Review clinical documentation for accuracy, completeness, and regulatory compliance

• Manage MDS transmissions and validate required submissions

• Coordinate and participate in interdisciplinary care planning

• Monitor Quality Measures (QM) and identify opportunities for improvement

• Collaborate closely with nursing, therapy, providers, and facility leadership

• Ensure resident care plans accurately reflect individual needs, goals, and services

• Support accurate reimbursement through proper assessment and documentation practices

• Maintain required MDS documentation and meet established deadlines

• Active and unrestricted RN or LPN license

• Minimum of 2 years of MDS experience in long-term care or skilled nursing

• Strong knowledge of the MDS and RAI process

• Experience with care planning and clinical documentation

• Knowledge of Medicare reimbursement and applicable long-term care regulations

• RAC-CT certification preferred

• Strong organizational, communication, and time-management skills

• Ability to effectively manage multiple deadlines and priorities
