Position Purpose
The Licensed Practical Nurse (LPN) – Charge Nurse provides hands-on nursing care and shift-level leadership within Annandale Village’s Skilled Nursing Facility, ensuring Villagers receive safe, compassionate, and person-centered care.
As the nursing leader on the assigned shift, the LPN – Charge Nurse coordinates nursing activities, provides direction and support to CNAs, responds to changes in Villager condition, and helps ensure care is delivered consistently and in accordance with individualized plans of care, physician orders, regulatory requirements, and Annandale Village standards.
This is a working nursing leadership position. The LPN – Charge Nurse remains actively involved in medication administration, treatments, assessments, documentation, and direct Villager care while providing visible leadership and clinical guidance to the nursing team.
Major Duties and Responsibilities
Providing Person-Centered Nursing Care
- Provide safe, compassionate, and person-centered nursing care within the LPN scope of practice.
- Administer medications, treatments, and prescribed therapies in accordance with physician orders and established procedures.
- Monitor Villager health status and promptly recognize, respond to, document, and communicate changes in condition.
- Perform nursing assessments, treatments, wound care, and other clinical responsibilities within the scope of practice.
- Advocate for Villager health, comfort, dignity, preferences, rights, and quality of life.
- Provide direct hands-on care as needed to support Villagers and the nursing team.
Leading the Nursing Team During the Shift
- Provide day-to-day direction, supervision, clinical guidance, and support to CNAs and other assigned nursing employees.
- Coordinate nursing activities and assignments during the shift to ensure Villager needs are met safely and timely.
- Lead shift huddles and communicate priorities, changes in condition, and other information necessary for continuity of care.
- Monitor completion and quality of assigned responsibilities and required documentation.
- Provide coaching, education, and real-time guidance to employees and support the orientation and training of new team members.
- Communicate employee performance concerns, staffing issues, and other significant matters to nursing leadership.
- Promote teamwork, accountability, professionalism, and compassionate care throughout the shift.
Coordinating Clinical Care and Medication Management
- Carry out physician orders related to medications, treatments, diets, and other nursing services.
- Maintain accurate medication and treatment records and ensure medications, controlled substances, and nursing supplies are appropriately managed and secured.
- Communicate with physicians and other healthcare providers regarding Villager needs, changes in condition, concerns, and treatment.
- Communicate appropriate health information and significant changes in condition to families and guardians in accordance with established procedures.
- Collaborate with members of the interdisciplinary team to support coordinated, individualized care.
- Participate in Villager assessments, care planning, admissions, transfers, and discharges as appropriate.
- Support staffing coverage and respond to call-outs and changing nursing needs, including providing support across units when necessary.
Quality, Safety, and Compliance
- Provide nursing services in accordance with applicable federal and state regulations, professional standards, organizational policies, and established procedures.
- Ensure nursing documentation is accurate, timely, complete, and reflective of Villager needs and services provided.
- Follow infection prevention, medication safety, emergency response, and other clinical safety practices.
- Identify and respond to concerns that may affect Villager care, safety, or quality of services.
- Participate in quality improvement, regulatory readiness, and other initiatives that support high-quality nursing care.
- Maintain Villager privacy and confidentiality and ensure Villager rights are respected.
Communication and Teamwork
- Maintain open, respectful, and timely communication with Villagers, families, guardians, healthcare providers, and team members.
- Promote effective shift-to-shift communication to support continuity and consistency of care.
- Work collaboratively with nursing leadership and other departments to support Villager health, well-being, and quality of life.
- Participate in staff meetings, interdisciplinary meetings, care planning, training, and other organizational activities as assigned.
- Foster a positive work environment centered on communication, teamwork, accountability, and person-centered service.
Other Responsibilities
- Maintain required licensure, certifications, training, and competencies.
- Follow Annandale Village policies, procedures, safety standards, and applicable regulatory requirements.
- Perform other duties in the best interest of Annandale Village and the Villagers we serve.
Required Qualifications
- Graduate of an accredited school of Practical Nursing.
- Current and unrestricted Licensed Practical Nurse (LPN) license in the State of Georgia.
- Three to five years of recent LPN experience in Georgia, with supervisory experience required.
- Ability to supervise and instruct CNAs and others and exercise independent judgment.
- Ability to respond effectively to emergencies and seek additional clinical support when appropriate.
- Ability to read and write in English, understand and follow written and verbal instructions, and communicate effectively with Villagers, employees, families, healthcare providers, vendors, and consultants.
- Strong leadership, training, goal-setting, and motivational skills preferred.
Employment Requirements
- Must be at least 18 years of age.
- Valid Georgia Class C Driver’s License, proof of insurance, and acceptable MVR.
- Must pass GBI and FBI background checks, including fingerprinting.
- Must pass drug screening and tuberculosis screening.
- Must maintain current LPN licensure and all required training and certifications.
Physical Requirements
- Ability to sit, stand, walk, bend, reach, kneel, stoop, and perform nursing responsibilities throughout the workday.
- Ability to provide hands-on nursing care, including medication administration, treatments, assessments, and emergency response.
- Ability to assist Villagers with mobility, positioning, transfers, and other care needs.
- Ability to lift up to 25 pounds independently and move up to 75 pounds with assistance.
- Ability to safely assist Villagers in and out of vehicles as required.
- Ability to perform CPR, First Aid, and other emergency response duties.
