• Provide direct patient care to a diverse population of residents, including those with complex medical needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct regular assessments and evaluations to ensure optimal patient outcomes
• Monitor and report patient condition to the interdisciplinary team and physician
• Participate in quality improvement initiatives to improve patient satisfaction and outcomes
• Collaborate with other healthcare professionals to ensure seamless transition of care

Qualifications:

• Education: Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN) from an accredited program.
• Licensure: Current and valid Registered Nurse (RN) license in New York. Experience: 1 year of experience in a skilled nursing or long-term care setting preferred, but not required.
• Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
