About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health or Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase and OASIS Recent experience within the last 6 months Must have reliable transportation and travel up to 75 miles Description: Provides coordinates and directs the provision of home nursing care according to physician's orders. Responsible for following agency policies and procedures through the competent application of the nursing process. Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and attends periodic office meetings. Travel will be required with the necessity of a current driver's license and agency-required liability insurance. Average patient load is 5-10 patients per day. Contractor will have responsibilities tied to patient care which equate to a point structure for weekly productivity. Types of cases include Start of Care, Wound Care, and Follow Up Visits. Must accept all patient assignments regardless of distance. Documentation must be completed within 24 hours after each patient appointment. There are no guaranteed hours, and the clinician must meet weekly productivity expectations. All overtime must be pre-approved. Must adhere to infection control precautions and coordinate care services. Proficient in conducting various nursing assessments and management tasks, including pain management and wound care. Candidates must not have any direct employment ties with any current agency or organization. Local candidates are accepted, provided travel commitments are understood.
