Registered Nurse Care Manager

Providence Health Plan Group seeks an Inpatient RN Care Manager to deliver patient-centered care at Covenant Hobbs Hospital in Hobbs, NM. This onsite role requires commuting to the hospital. Responsibilities include assessment, care coordination, discharge planning, documentation, regulatory compliance, and patient advocacy to ensure the right care at the right time. Required: Associate’s Degree in Nursing and NM RN licensure; 2 years acute care nursing in Medical-Surgical or inpatient settings. #J-18808-Ljbffr
