PrimaryOne Health is currently seeking an OB Intake Nurse for our Columbus Health Centers.

Essential Job Responsibilities:

• Meets with each new OB patient to determine the level of care needed.
• Educated OB patient on delivery hospital, GBS documentation, breast/bottle feeding, postpartum care.
• Works collaboratively with the OB health care team including providers, medical assistants, social workers, and nursing team.
• Identifies and coordinates referrals to community resources in regards to patient need.
• Completes documentation of all professional activities, contacts and observations promptly and appropriately.
• Attends internal and community meetings as assigned and participates in Quality Improvement activities.
• Occasional travel required.
• Other duties or special projects as assigned

Successful candidates will have a LPN license and 2-3 years of OB clinical care experience.
