divh2Case Management Assistant/h2pAssists the Case Management staff by performing clerical duties related to case management and discharge planning. Has full access to patient health information./ppAssists with coordination and completion of forms including but not limited to:/pulliFL-2 forms - CM staff review and doctors signature for extended care facility placements/liliImportant Message from Medicare - obtaining patient/family signatures prior to discharge/liliAs needed, takes documents to physician office for signature and/or delivers document to post-acute providers/li/ulpAssists with coordination of information such as:/pulliDischarge date, disposition, discharge instructions/orders and/or discharge summary to post-acute providers/liliCommunicates pertinent patient information to post-acute providers and 3rd party payers./liliObtains medical records for submission to receiving facilities, 3rd party payers, and QIO as requested by Care Coordinators and Social Workers./li/ulpMaintains (keeps updated and/or well-stocked) computer and paper files including but not limited to:/pulliResource Boxes on each unit/liliInformation Booklets/liliDocuments in Shared Drive/li/ulpAssists with data collection and statistical report preparation for:/pulliDepartmental Quality Improvement/liliQuality Coordinating Council/liliUtilization Review Committee/li/ulpAssists with timely Departmental Communication by:/pulliTaking telephone messages/checking voicemail for the Case Management department/liliReceives fax and printer communications and conveys to the appropriate person(s) in a timely./liliEMS Convalescent/liliTaxi/liliWheelchair transport/liliCare Transitions Clinic Appointments/liliCommunity Clinic Appointments/li/ulpAssists with other tasks or duties as requested by the Director of Case Management and Care Transitions/ppShift: Saturday and Sunday 8:00 AM - 4:30 PM/ppRequirements/ppHigh School Diploma or equivalent required/ppValid North Carolina Drivers license/pp1-2 years clinical and/or clerical experience preferred/ppKnowledge of medical terminology required/ppKnowledge of operation of personal computer and Microsoft Office Suite/ppMust possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds. Visual acuity, hearing, stooping, reaching and kneeling./p/div
