At H2 Health @ Berea, you’ll work in a 2,500-square-foot outpatient clinic alongside an experienced therapy team with more than 80 years of combined experience. The clinic serves a diverse general outpatient population and offers programs across orthopedic and musculoskeletal rehabilitation, balance and dizziness, sports medicine, chronic pain, neurological conditions, manual therapy, Workers’ Compensation, and other rehabilitation needs. Your Role Evaluate patients and develop individualized, evidence-based Physical Therapy plans of care Treat a diverse outpatient caseload focused on mobility, strength, function, and return to activity Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training Educate patients on movement, recovery, injury prevention, and home exercise programs Collaborate with clinicians and support staff to coordinate high-quality care Track patient progress and adjust treatment plans to support meaningful outcomes Complete timely and accurate clinical documentation Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program Current Kentucky Physical Therapist license or eligibility for Kentucky licensure CPR certification, if applicable Strong communication and collaboration skills Commitment to high-quality, patient-centered care New graduates are welcome to apply; previous outpatient experience is not required Full-Time team members may include: Performance-based bonus opportunities tied to productivity, caseload growth, revenue generation, and clinical contribution Medical, dental, and vision insurance PTO and paid holidays 401(k) with company match Tuition loan reimbursement HSA/FSA options Employee Assistance Program Company-paid life insurance Additional voluntary benefits Part-Time and PRN team members: 401(k) with company match Workplace Experience & Perks Monday–Friday outpatient environment Flexible scheduling may be available Full-Time, Part-Time, and PRN options Access to Scribe AI-powered documentation technology to help reduce documentation burden Manageable caseload expectations with strong administrative support For most Full-Time outpatient clinicians, H2 Health generally targets approximately 10–12 patient visits per day as part of the clinician experience H2U continuing education and live CEU opportunities Professional growth and leadership development opportunities Employee recognition programs Career advancement opportunities across H2 Health Collaborative culture that values initiative, ownership, and clinical excellence H2 Health is proud to be an Equal Opportunity Employer.
