Hospice weekend RN

Position Overview Full-time Home Care Baylor Registered Nurse with St. Mary Home Care & Hospice (Trinity Health at Home) in Athens, GA. Shift: Three 12-hour shifts (paid 40 hours). Works Saturdays & Sundays with a Friday or Monday included. Sign-on Bonus: $10,000 (not applicable to internal employees, former employees who resigned in the last year, or applicants sourced from outside firms). Key Responsibilities Provide primary nursing care to home-based care patients as ordered by the physician, in accordance with organizational policies and procedures. Conduct initial assessment and develop care plan in collaboration with the Population Health Case Manager. Provide functional support/supervision to Home Healthcare Aides and LPNs as needed. Deliver therapeutic intervention, overall care management, and achieve top decile client outcomes and patient satisfaction. Qualifications Graduate of an approved nursing education program. Registered Nurse licensure in the state of PA. One (1) year experience as a professional care nurse. Current driver's license and reliable transportation.
