Job Description Wilshire is offering a $5,000 Sign on Bonus for Full-Time Positions Home Health Registered Nurse Position (Full time, Part time and Per Diem positions available) Open Positions located in San Luis Obispo County and Northern Santa Barbara County. We’re looking for motivated professionals to join our team of Registered Nurses to care for our community. Under the direction and supervision of the Clinical Manager, the Home Health Registered Nurse is responsible for the comprehensive nursing care of assigned patients which includes recognition of emotional, social and cultural factors, encouragement of self‑respect and self‑esteem, guidance toward rational and socially acceptable behavior and implementation of restorative nursing care measures. Full‑time Registered Nurse will act as Case Manager for specific assigned cases, coordinating patient care, and collaborating with other disciplines involved in the patient’s care and for supervision and training of nursing personnel who assist in provision of care to the assigned patients. Responsibilities Conducts initial evaluation visit and submits completed paperwork in a timely manner. Responsible for the admission process and orientation of patients. Completes full body assessment on each patient during initial visit. Assesses the total needs of the patient and develops nursing care plans to meet those needs. Initiates appropriate preventive and rehabilitative nursing procedures. Initiates and revises the plan of care as needed, clarifying new orders and discontinued orders. Prepares clinical and progress notes. Responsible for accurate observations, evaluations and reporting of symptoms, reactions and progress of patients to the physician and Clinical Supervisor, and others participating in the care and treatment of the patients. Qualifications and Requirements Graduation from an approved school of professional nursing, BSN preferred. Current licensure as a Registered Nurse in the State of California. Demonstrated knowledge of and ability to apply basic principles of nursing care. Experience and/or education in the area of the aged, chronically ill, restorative nursing, and/or psychiatric nursing. One or more years of experience as a Registered Nurse is required; Med‑Surg experience preferred. Apply today; this could be the moment that changes your life!
